New Patient Information Sheet

Thank you for choosing the Lymington Chiropractic Clinic.

On your initial contact with the reception team you will be asked a few questions regarding your name, contact details, date of birth etc. to enable us to set you up as a new patient. All patient details are private and confidential and will not be shared with any third parties.

As part of the new patient procedure, you will be asked for an email address. This is to allow us to send you a short questionnaire (from Forms4Clinics) for you to complete and return electronically in advance of your first consultation. Forms4Clinics is a free and pragmatic system to help practices gather and report clinical outcome and patient satisfaction information, if you do not wish to give us your email address a paper copy of the questionnaire can be completed at the clinic.

An initial consultation with one of our chiropractors lasting about 1 hour will be booked for you. An SMS text (if a mobile telephone number has been given), or a telephone call from the reception team will remind you of your appointment details in advance.

We ask patients to arrive 10 minutes early for the initial consultation to enable a patient consent form to be completed here in reception and for the Forms4Clinicsquestionnaire to be completed if not already done so electronically.

When you meet the chiropractor for the first time, they will have a discussion about you, your health and your reasons for wanting chiropractic care. Any health concerns highlighted on the Forms4Clinics form will also be included in the discussion with you, concerning your health.

Your medical details are also confidential. No one else will be allowed to see your notes without your permission.

Following the discussion, a physical examination normally takes place. The examination will be thorough and cover all concerns expressed at the time of the initial conversation with the Chiropractor. We may need to ask patients to wear shorts or gowns if necessary. These are available at the Clinic.

Once the examination is complete, the chiropractor will explain the findings of the examination and how they relate to your condition and symptoms.

If treatment is appropriate, the chiropractor will then discuss this with you and explain how that will proceed. If there is a need for a referral for an x-ray or possibly to someone who may be better able to help, then we will let you know that as part of the initial consultation.

If you are happy to proceed and it is safe to do so we will treat on the first visit, we usually do!
